MESSY CHURCH

Messy Church is an opportunity for children, families, and adults to engage in hands-on creative activities, celebratory worship, and time together in community.

CHILDREN'S CHOIR

First Church's Children's Choir enthusiastically welcomes all children in grades 1-6, regardless of musical background. Weekly Wednesday rehearsals start September 13 from 5:15-6:00 p.m. in the 3rd floor Choir Room . An adjacent room is made available for parents and caretakers to relax and enjoy fellowship during rehearsals. SUMMER CAMPS

First Church offers a variety of summer camp experiences for children of all ages and families of all makeups. Camp registrations open early 2024. Check the boxes below to receive registration updates as soon as they're available.

All children who participate in ANY Children's Ministry programming, whether within the church building or at events which may be held offsite (Ex.: Club123, J.U.M.P., etc.), must have a completed current Master Medical Form on file prior to their attendance at any such event. 

This form will be kept on file with the church for the program year 2023-2024. If any of the medical information should change within this year, please submit a new form and/or notify Colin McDonald at [email protected] .

ALLERGIES and OTHER MEDICAL CONDITIONS

The following information must be filled in by the parent/guardian.  The intent of this information is to provide our staff and volunteers with background to provide appropriate care.  Any changes should be communicated with us as updates are necessary.   

List all known allergies to food, medication, or other allergens.  Describe reaction and management of the reaction.  

Please also list and explain any medical conditions which staff should be aware of.

MEDICAL CONSENT

I grant permission for my child to receive any emergency medical treatment and/or transportation for medical/hospital treatment while participating in classes, worship activities, fellowship groups, camps, mission trips, retreats, and other programs with First United Methodist Church, 516 Church Street, Evanston IL 60201, 847-864-6181.

First Church's Children's Ministry is enriched and made possible by the support and involvement of a diverse group of lay ministry leaders, including parents and caregivers. Please check the box below if you are able to consider serving one Sunday a month as a teacher, assistant, or "behind the scenes" helper. You will be contacted by a member of the Children's Council to determine a schedule that suits your interests and availability. Thank you!

First United Methodist Church of Evanston, IL may use current or prior year's photographs and/or videos of and quotations from our children in our print publications, website and social media pages. These photos, videos and quotations are never associated with the full name of the child. Photos, videos, and quotes are used solely to promote and support our ministry.

Waiver and Release of Liability

I agree that First United Methodist Church Evanston, its staff and volunteers, will not be held responsible for accidents arising from my child's participation in on and offsite church activities. In the event that my child's parent/caregiver and/or emergency contact cannot be reached in case of an emergency, I hereby grant permission to the physician selected by the church to secure and administer treatment. I agree to indemnify and hold harmless First United Methodist Church Evanston, its staff and volunteers, against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf.
